Supports For Community Living
The SCL Waiver services are designed to serve individuals with Intellectual and Developmental Disabilities who meet the criteria to reside in an Intermediate Care Facility but choose to receive services & supports within the community. The SCL Waiver provides support for services designed to assist with community-based residential care, including care in Staffed Residences and Adult Foster Care homes.
FAMILY HOME PROVIDER (FHP)
Non-Traditional contractors that provide home environment to participants. This can include washing clothes, feeding, bathing, taking participant to appointments, managing community integration, etc.
RESPITE
A service designed to relieve care givers/guardians (non-paid supports that the participant lives with) for a specific time period.
CA (COMMUNITY ACCESS)
Promotes involvement in clubs and organizations including educational, recreational, civic, and volunteer activities to build ongoing independent relationships with others in the community.
Eligibility

Enable a person to accomplish tasks that the person normally would do for himself or herself. If he or she did not have a disability. These include hands-on assistance, reminding, observing, guiding or training during an activity; assistance in managing medical care and transportation to access community service, activities to appointments. If these are not available under the Medicaid Program, Personal assistance services are available only to those who live in their own residence or in their family’s residence.

You must first obtain financial eligibility for Medicaid. You can apply for Medicaid online using kynect, by calling the Department for Community Based Services (DCBS)at (855) 306-8959 or in-person at a DCBS office. There are some documents that are helpful to have when applying. The Medicaid Waiver Services Fact Sheet lists those documents.
